External diameter > 5.0 cm

External diameter > 5.0 cm

Or at < 5.0 cm if:

Growth at greater 0.5 cm/yr

Or at 4.6-5.0 cm if:

Dilatation at greater 0.2 cm/yr

Or

Family history dissection at <5.0 cm

Or

Family history dissection

Or

Presence of significant aortic regurgitation

Or

Severe aortic or mitral regurgitation

Or in women contemplating pregnancy

>4.0 cm

Or

Pregnancy is being planned#

Also consider if:

Maximum CSA/height in metres >10 *

In ‘small’ individuals use:

Indexed diameter adjusted for BSA of 2.75 cm/m2[58]

  1. CSA = cross sectional area in cm2 of ascending aorta or root BSA = body surface area.
  2. * Shorter patients have dissection at a smaller size. 15% patients with MFS have dissection at aortic root diameters less than 5.0 cm.
  3. #Consider at 4.0-4.5 cm depending on serial growth on imaging and family history [59].
